- Save the form and close it. Send the form via email to sn.ihc@polyu.edu.hk with title "Certified Laughter Yoga Leader Training (Class III)".
- Settle the payment with a cheque/ bank draft payable to “The Hong Kong Polytechnic University" within 5 working days of registration and send it to the following address; please write your name, phone number and reference number (if any) at the back of the cheque. Do not send cash.
